The Method Weight loss

How should you strength train on GLP‑1 medication?

Lift at least three times a week, take sets close to failure, eat enough protein, and track your strength, not only the scale.

What most people miss

The medication changes how much you eat. It does not decide what the weight that comes off is made of. A 2025 advisory from four medical and nutrition societies puts preserving muscle and bone through resistance training and appropriate diet among the priorities during treatment, with strength training at least three times a week and enough protein, and says protein without the lifting is likely not enough.1

The same advisory starts with an assessment of muscle strength, function and body composition, not only weight.1 The scale cannot tell you what you are losing. Strength logged set by set is one of the few numbers you can watch every week.

Hard sets are the ones that count. Muscle growth rises as sets end closer to failure,2 and most people stop further away than they think: in 141 people, the typical error in calling reps left was about three.3

Volume can give; the lifting should not. In trained men eating in a deficit, without medication, three sets per exercise kept as much lean mass over six weeks as five did.4 So on a week when appetite and energy are low, the answer is fewer hard sets, not a skipped session.

Protein is the other half. For lean people training in a deficit, the research points to 2.3 to 3.1 g per kilogram of lean mass, with few studies behind the range.5 Your own number is a question for a registered dietitian, and the advisory names dietitian counselling among the supports.1

  1. Mozaffarian, Agarwal, Aggarwal et al., 2025 · Am J Clin Nutr

Joint advisory · four medical and nutrition societies

Covers GLP-1 drugs and their combinations. Strength training at least three times a week alongside the drug, with enough protein. Protein without the lifting is likely not enough.

  2. Robinson, Pelland, Remmert et al., 2024 · Sports Med

    Meta-regressions

Muscle growth rose as sets ended closer to failure; strength gains were similar across reps in reserve. Exploratory: reps in reserve were estimated from the studies.

  3. Steele, Endres, Fisher et al., 2017 · PeerJ

    n = 141

People underpredicted their reps to failure, with a typical error of 2.6 to 3.4 reps.

  4. Roth, Schwiete, Happ et al., 2023 · Scand J Med Sci Sports

Randomised trial · trained men · 6 weeks in a deficit

Three sets per exercise against five, eating 30 kcal per kg of body weight: no difference in lean mass or muscle thickness.

  5. Helms, Zinn, Rowlands, Brown, 2014 · Int J Sport Nutr Exerc Metab

Systematic review · 6 studies · lean, trained, dieting

2.3 to 3.1 g per kg of lean mass, higher the leaner you are and the bigger the deficit. Few studies, so read the range as a guide.

From the floor

Tell your coach you are on one. Every session is coached and every set logged, so your strength is tracked beside the scale, and food targets go to the registered dietitian in the building.
